Declaration of consent
Introduction
Sanos Clinic collects health information to help find new therapies.
The purpose of our collection of data and the processing of your personal information is solely to improve health, and your personal data will only be used for this purpose. By signing up for Sanos Clinic’s database, you give Sanos Clinic permission to collect, store, systematize and pass on both your contact and health information to scientific institutions (private and public institutions with ongoing research projects, which are always approved by the Institutional review board (IRB).
Any profile may at any time be deleted by Sanos Clinic at your request.
By signing up for the database, you agree to receive email, SMS and phone calls from Sanos Clinic or it's scientific partners.
If you no longer wish to receive emails, SMS or phone calls, you can unsubscribe from the service at any time.
Questionnaire and clinical studies are not mandatory, and you may decline to participate at any time. Storage and other processing of your information in our database is encrypted.
By creating a profile, you consent to the Sanos Clinic to collect a collection of personal data from you for the purpose of processing these to pursue the above-mentioned purposes, i.e. contacting you regarding possible participation in a questionnaire or in a clinical study. The personal data will include the following:
- Contact Information
- Age
- Place of residence
- Weight
- Height
- Medical History
- Medicine
- Dietary habits, including alcohol and nicotine consumption
The terms and conditions for this consent and your rights in connection with the consent are given below.
By submitting this form you agree to all the terms of the consent. Any profile can be deleted at any time. Deleting your profile will permanently delete all data.
If there are changes in our terms, you will receive an email about this. You subsequently have the option of either accepting the changes or deleting your profile.

3. Purpose and legal basis for processing your personal information
We process your personal information for the following purposes:
Sanos Clinic collects your personal information to determine whether a notice of possible participation in a questionnaire or clinical study must be submitted to you.
It is noted that the terms of any participation in a clinical study or questionnaire survey, including consent to collecting your personal information for that purpose, will be stated in a separate statement of consent that you will receive if and if you wish to attend.
If you are considered as interesting in relation to a possible participation, we will contact you by e-mail, SMS or phone call.
The legal basis for our processing of your personal data is the consent you have given in connection with your creation of a profile in the Sanos Clinic database.
4. Categories of personal data
We treat only the categories of personal information about you listed in the introduction to this consent statement.
5. Where your personal information originates
The personal information about you being treated originates solely from you and is thus obtained when you create a profile in the Sanos Clinic database.
6. Retention of your personal information
We keep your personal information as long as your profile is registered in our database. If you delete your profile, your personal information will also be deleted.
7. The right to withdraw consent
You have the right to withdraw your consent at any time. You can do this by contacting us at the contact details listed in section 1 above.
If you choose to withdraw your consent, it does not affect the legality of our processing of your personal information based on your prior consent and until the date of withdrawal. If you withdraw your consent, therefore, it will not be effective until this date.
8. Your rights
You have a number of rights under the Data Protection Regulation in relation to our processing of information about you.

Right to see information (right of access)
You are entitled to gain insight into the information that we are processing about you, as well as a number of additional information.
Right to rectification (correction)
You are entitled to receive incorrect information about yourself.
Right to delete
In special cases, you have the right to delete information about you before the time of our general deletion occurs.
Right to limitation of treatment
You may have limited access to the processing of your personal information. If you are entitled to limited treatment, in future, we may only process the information - except for storage - with your consent or for the purpose of determining, enforcing or defending legal claims, or for protecting a person or important social interests.
Right to object
In some cases, you may object to our or legitimate processing of your personal information.
You can also object to processing your information for direct marketing.
Right to transmit information (data portability)
You may, in certain cases, have the right to receive your personal information in a structured, commonly used and machine-readable format, and to transfer these personal data from one data manager to another without any obstacles.
